How to Request a Lawrence Death Certificate
Lawrence death certificates cost $10 each. You can request them in person or by mail. There is one critical restriction: for safety and protection of sensitive records, the Office of the City Clerk will NOT respond to requests for specific record information by telephone, email, or fax. You must either come in person or send a written mail request.
In person, visit Room 107 at 200 Common Street during office hours. Bring the name and date of death for the record you need, along with valid ID and payment.
For mail requests, follow these steps. First, write a letter with the name and date of death for each person whose record you need. Second, include payment of $10 per certificate as a check or money order payable to The City of Lawrence. Do not send cash by mail. Third, include a self-addressed pre-stamped envelope with enough postage. If you need overnight delivery or return receipt service, prepay those costs in advance. For records from 1926 onward, the state RVRS at 150 Mount Vernon Street, Dorchester also holds copies and can issue them by in-person visit ($20), mail ($32), or online through VitalChek ($54 for the first copy).
